Can anyone answer these questions about defibrillators?

Reading about woman who just died in the American Airline airplane and whom her cousin says was not properly helped with the oxygen and DEFIBRILLATOR, it’s prompted me to ask about this machine which I’ve been hearing about for a few years now.

There are two types of defibrillators. You can have them implanted in you (internal defibrillators) with a wire that runs into your heart and controls its contractions. Or you can have the ones that are put in public places (external) that can be used to shock people that have a heart attack. Most recently the AED (automatic external defibrillator) was introduced as a much simpler defibrillator that trained people can easily use and would be protected by the Good Samaritan law.
There are tiny electrical signals (on the level of millivolts) that run through your heart and tell it when to contract. When you are healthy these electrical signals go through your heart in a rhythmic way, thus resulting in rhythmic contractions that push your blood through your body and allow your body to function as normal. If your heart is weak or old, these electrical signals can get disrupted, and they can go crazy and out of rhythm. This causes your heart to beat irregularly (arrhythmia). What a defibrillator does (external or internal) is deliver a much bigger level of electricity to your heart so as to interrupt these crazy electrical signals and get the electrical signals back into rhythm.

To answer your specific questions:
How truly effective are they? Internal defibrillators, otherwise known as pacemakers, are very effective and are implanted in thousands of people each year. External defibrillators are only effective if the person receiving treatment has an arrhythmia, meaning the crazy electrical out of rhythm signals I was talking about earlier. If youre in the mall and someone crashes to the floor grasping their chest like theyre having a heart attack, you are not going to be able to tell if theyre having an arrhythmia or some other heart condition. So using an AED to shock their heart is not going to be effective if they have an arterial blockage or some other non-electrical condition.

Why not require that most public places have them? A few reasons. First, a business that has them in their building may have to train their employees to use them. A lot of people are uncomfortable delivering electric shocks to people that are lying on the floor having a heart attack. A lot of people are uncomfortable handling a device that delivers so much electricity. If businesses don’t train people how to use these devices, someone may use one that doesn’t know how and could end up injuring themselves or others. So external defibrillators aren’t put in a lot of public places is because of the cost to train people to use them and to avoid lawsuits. However, in a lot of places laws are being made to protect people with the good Samaritan law, should something go wrong with use of the AED.

Are they recommended for most heart patients or only those diagnosed as probable heart attack victims?
For pacemakers and internal devices, they are given to people because they have electrical signaling problems. For the AED, it is pretty much used on anyone grasping their chest and having the symptoms of a heart attack, although like I mentioned before this is difficult to tell based on looking at the person.
Do any INSURANCE or MEDICARE cover them? For internal defibrillators or pacemakers, yes. Not sure about the external ones, as those are assigned to a business and not a specific person.
